Scheduling an Appointment
Please contact your preferred imaging center to schedule an appointment. A referral slip from your referring doctor is required, as California law requires a prescription before X-rays can be performed.

Walk-ins are welcome when availability permits! Same-day or next-day appointments are often available. Please call ahead and bring your referral slip to your appointment.

ELDERLY PATIENTS POLICY
Patients who require assistance must have a caregiver remain on-site throughout the appointment. If needed, staff will assist the patient to the building entrance and remain with them until their transportation arrives.
PATIENTS UNDER 16 YEARS OF AGE POLICY
Patients under 16 should be accompanied by a parent or guardian. Parents may accompany their child but must remain outside the X-ray room and any radiation exposure areas during imaging.
Billing & Insurance
Payment
Payment is due at the time of service unless your referral is marked “Bill Doctor.” We accept cash, checks, Visa, Mastercard, American Express, Discover, and CareCredit financing.

Dental Insurance
C-Dental X-Ray and McCormack Dental Imaging are out-of-network with dental insurance providers. As a courtesy, we can assist with filing your insurance claim using the appropriate procedure codes. Any reimbursement is paid directly to you by your insurance company, and you are responsible for following up with your insurer.

Claims mailing address (usually listed on the back of your insurance card)
Medical Insurance
Medical insurance claims require a referral from a DDS or MD and a medical diagnosis code provided by your doctor. We may assist with filing claims for eligible imaging services related to dental treatment, including restricted airway, TMJ, and oral surgery.
You are responsible for following up with your insurance company regarding your claim.
